Transfer Measurements

Use this guide to prepare preliminary measurements for transfers involving beds, chairs, wheelchairs, toilets and other daily-care situations. It can be used for children or adults.

You do not need to complete every field. Enter “Not sure” or “Unable to measure safely” where appropriate.

These measurements support SafetyAid’s equipment review but do not confirm final suitability, lifting method or transfer safety.

Do not perform a difficult transfer just to take measurements

Measure surface heights, distances and available space without lifting or transferring the person when this cannot be done safely. Do not attempt a transfer beyond the caregiver’s usual ability.

Stop if the person or caregiver feels unsafe, unstable, uncomfortable or at risk of falling. Record “Unable to measure safely” and request assistance when needed.

Preparation

Before you start

  • Use centimetres unless the guide states otherwise.
  • Measure surfaces while they are stable and safely accessible.
  • Lock wheelchair or equipment brakes where normally required.
  • Keep floors and transfer areas clear.
  • Ask another adult to assist when one person cannot measure safely.
  • Do not lift, pull or reposition the person only to obtain a measurement.
  • Record “Not sure” rather than guessing.
  • Note the person’s usual transfer method and support.
Safe preparation

Measure the environment first

Surface heights

Measure the bed, chair, wheelchair, toilet or commode from the floor to the surface normally used during transfer.

Transfer distance and available space

Measure the gap and clear working area without performing the transfer. Include obstacles, doorway limitations and wheelchair space where relevant.

Assistance needed

Describe the person’s usual assistance, caregiver support and transfer equipment. Do not test a more difficult transfer for this form.

Unable to measure safely

Leave the value blank or record “Unable to measure safely.” SafetyAid may request an alternative photo, video, professional observation or later measurement.

A family member or caregiver may prepare preliminary transfer measurements. Complex lifting, balance, weight-bearing, pain, tone or caregiver-safety concerns may benefit from input from a physiotherapist, occupational therapist, rehabilitation professional or trained transfer specialist.

Professional involvement is helpful where needed but is not required to begin a SafetyAid assessment.

This guide supports equipment and transfer-environment preparation only. It does not provide medical diagnosis, treatment, manual-handling training or clinical assessment.
